New patients must complete the Adrenal Fatigue Form, the Thyroid Hormone Evaluation Form, Diet Log form and either the Female medical history evaluation form or the Male BHRT evaluation form which ever is gender appropriate.   Please fax or mail your forms to us well in advance of your consultation appointment.
